I am the legal guardian of the child(ren) and hereby give permission for my child(ren) to travel on transportation to Camp Calvary to participate in the Cornmaze on Saturday, October 7, 2023. I hereby authorize Calvary Baptist Church (CBC) to secure any emergency medical treatments necessary and waive and release CBC and its directors, agents, and employees from any and all liability, claims and demand for any personal injuries, sickness, death, or damages of any kind sustained by my child, as well as any expenses of any nature whatsoever which arise out of or are related to my child(ren)’s participation in said activity. I further understand that CBC does not provide health insurance coverage for my child.
