I do hereby release Calvary Baptist Church and it ministries, its directors, employees, agents, and all organizers and helpers from any and all liability, claims, or demands for personal injury, sickness, or death. I further understand that Calvary Baptist Church does NOT provide health insurance coverage for my child. I understand that my child(ren) may be photographed or video recorded, either individually or with other participants or attendees, at such activities or events. I consent to my child(ren) being photographed or video recorded, and I consent to the use of any such photographs or video for any legtimate purpose by Calvary Baptist Church and the organizers of this event.

In a world of shifting sands, God's Truth stands firm like a rock! Join us for Breaker Rock Beach EBF this year, running June 24-28 from 6:00-8:30 each night.
Kids who are entering grades 1-6 in the 2024-25 school year are invited to join us all week as we play, sing, snack, learn, and study God's Word together. Please fill out one form for EACH of your children attending.
